YCF was originally incorporated as Allen County Hospital Foundation to support the work of the Allen County Hospital which, at that time, was leased to Health Midwest, a not-for-profit hospital corporation based in Kansas City, Kansas and Missouri. In March of 2003, Health Midwest sold all of its hospital assets to the Hospital Corporation of America (HCA), a for-profit hospital corporation. Because the Allen County Hospital Foundation was a tax-exempt 501(c)(3) corporation, it could no longer directly support the operations of the Allen County Hospital while it was leased to HCA. As a result, in December of 2003, the Allen County Hospital Foundation amended its Charter to change its name to Allen County Health Care Foundation (ACHCF) and dedicated its ongoing work to serving the broader health needs of Allen County.

When the new Allen County Regional Hospital project got underway in 2010, the ACHCF served as the charitable organization through which fund raising for the new hospital took place. During the hospital capital campaign, over $2 million dollars was raised by numerous community volunteers to purchase equipment for the new hospital.
